Appointment Visit Check List

To ensure a smooth appointment process, please ensure you have reviewed our patient appointment checklist:

Please bring the following to your appointment:

• Completed patient intake forms

• Government-issued ID
• Medical insurance card
• Form of payment
• Medication and allergy list 
• Any Imaging studies on CD/DVD

Day of your appointment:

• Please arrive 15-20 minutes early to ensure a smooth check-in process.

Appointment Cancellation Policy:

• Office Consultations & Follow-Ups: Please provide at least 24 to 48 hours’ advance notice; scheduled surgical procedures require five business days’ notice. Cancellations outside this window, or missed visits, typically carry a $50 to $100 late-cancellation/no-show fee.

• If you need to reschedule due to an acute illness or medical emergency, please contact our office as soon as possible so we may release your time slot to another patient in need.
